Delta and American Airlines require a section of your travel packet to be completed by a veterinary professional to ensure safety for travel with your ESA. You can have this form completed by your own vet, or through one of our own. Please note that you must have proof of rabies vaccination for this service to be completed. Failure to provide proof of your dog's rabies vaccine will result in a denial, and non refundable evaluation $99 evaluation fee.
